a list of suspected double numbered patients is identified
through computerized demographic matching, the suspect charts
are pulled and analyzed for patient identifying matches. Confirmed
matches are combined into a single medical chart and duplicate
numbers are indexed to point to the combined chart.

File Matching
Suspect

Master Patient Index is updated to point to a single medical
record number containing all patient information and history.
Depending on your Master Patient Indexing method, retired
patient identifier numbers can be purged from your system
or cross referenced to the patient's single medical record
number.
